Community forensic outreach service

This program works with clients from the point of a finding by the court of Incompetent to Stand Trial or Not Guilty by Reason of Insanity. The Forensic Mental Health Service aims to provide timely and responsive forensic mental health services to meet the needs of mentally ill persons who have, or are at risk of involvement with the Criminal Justice System (CJS ). The forensic outreach service provides aftercare and support for men and women aged and over who are returning to the community from forensic inpatient care.

It’s provided for those people who are discharged from inpatient forensic services and for those who are referred from other sources. Community safety is also an important goal. We’re also involved with the rehabilitation and preparation for discharge for people receiving care in the inpatient forensic services.

The team provides risk assessment, formulation, and management advice to local inpatient and community teams, for service users with enduring mental health conditions who present a risk to others. We also offer support to their carers. We co-ordinate the care of people in the community who are under a restriction order of the Mental Health Act.

Referrals can only be accepted from secondary care mental health and learning disability services, high secure hospitals or other secure units. Referrals to the FIND service need to be made using the referral form of the Forensic Outreach Liaison Service.

About the service We currently provide advice and consultancy until the service is fully operational. The service provides follow up and interventions to service users discharged from low and medium secure services. The service also provides liaison and consultation services to local psychiatric services in ELFT and NELFT localities. When a client with this legal status is eligible for release from the state hospital, he or she is placed on Conditional Release (CR).

Our Forensic Outreach team works with the Provincial Health Services Authority (PHSA) Forensic Regional Clinic team, providing outreach services to individuals experiencing concurrent mental health and substance use challenges. Through flexible and person-centred outreach services, our Forensic Outreach program promotes public safety while meeting the needs of the individuals they are supporting. People are referred to our service by the Courts, local authorities, probation services, other health professionals.

Outreach : Outreach activities are designed to provide information and resources to help victims of crime and sexual violence. The VSC reaches out to community partners through small group presentations, tabling events, our billboard campaign, a monthly newsletter, and collaboration with other community organizations. They may also be subject to Home Office restrictions, Probation Orders or have judicial proceedings outstanding. Forensic Treatment Treatment Services help individuals with mental health, substance abuse, or co-occurring disorders attain sufficient stability of lifestyle and successful involvement with lesser intensive community programs or self-sufficiency based on individually designed interventions and clinical skill building.

The community outreach team delivers crime prevention, educational and victim services directly to Miami Dade’s residents, schools, faith, civic and neighborhood organizations and business groups. The specialist forensic outreach team provides a community based service to service users who are still under the care of specialist forensic psychiatry.

The team is based at the Bretton Centre at Fieldhea Wakefiel covering the Kirklees and Calderdale areas. We provide a service that monitors health and social welfare as well as monitoring mental health and managing risk. There were eight community forensic workers ( community psychiatric nurses or social workers), forensic psychiatrists, four service managers, four forensic psychologists, and four general psychiatrists with experience in managing forensic patients. Twenty-nine participants (percent) completed the first round of consultation.

It is primarily for patients who require a specialist forensic package of aftercare and usually subject to monitoring by the Ministry of Justice following a conditional discharge. The service will also work with young people in transition from children’s services into adult services. This service provides a community focus for supporting people who have committed a criminal offence while mentally unwell.
